How to Enrol
To enrol as a patient at Peninsula Medical Centre you will need to complete the two forms below. Please bring your completed forms into reception.
The Ministry of Health now requires proof of eligibility for funding. For all patients registering at this practice, please bring your New Zealand or Foreign Passport for proof of citizenship, permanent residence, or work permit.
